The Family Practice Nurse (FPN) is a Licensed Practical Nurse who will work collaboratively within a family practice environment to enhance the delivery of primary care services in alignment with the Patient’s Medical Home Model of Care. The Family Practice Nurse will provide a broad range of services, including but not limited to, health education and promotion, management of routine screening maneuvers, chronic disease prevention and management, acute episodic care, therapeutic interventions, well-woman/well-baby care, and care coordination. The Family Practice Nurse will be a complementary source of support to the physician providing wound care, injections, and other clinical interventions to patients as directed by the physician. The Family Practice Nurse will have scheduled patient appointments and will be available as needed for non-scheduled patient encounters.
