Cold Lake is in northeastern Alberta and is named after nearby Cold Lake. Canadian Forces Base Cold Lake is situated within the city’s outer limits.

Cold Lake was first recorded on a 1790 map, by the name of Coldwater Lake. Originally three communities, the City of Cold Lake was formed by merging the Town of Grand Centre, the Town of Cold Lake, and Medley (Canadian Forces Base 4 Wing) on October 1, 1996. Grand Centre was renamed Cold Lake South, and the original Cold Lake is known as Cold Lake North. Because of its origins, the area is also known as the Tri-Town. The City of Cold Lake has a population of approximately 16,000 people.

About The Town of Bonnyville

Bonnyville is a town in east-central Alberta, Canada between the City of Cold Lake and Town of St. Paul, and is located on the north shore of Jessie Lake. The Town of Bonnyville has a population of approximately 6,500.

About The Cold Lake PCN

The Cold Lake Primary Care Network was formed in 2010 to help support the Family Physicians deliver Primary Healthcare Services to the local communities. The Cold Lake PCN is a Nurse Practitioner lead clinic that provides care to those who are not attached to a Family Physician.

About The Bonnyville PCN

The Bonnyville PCN’s goal is to provide practical healthcare solutions by giving you access to the medical services you need without the complication of visiting many hospitals or clinics.

Position Summary

This is a shared position within the Bonnyville and Cold Lake Primary Care Networks. Candidates will have the opportunity to work a hybrid schedule: on-site within both the Bonnyville and Cold Lake Primary Care Network clinics and remotely.

The Practice Facilitator is a natural relationship builder between member physicians, their clinic teams, and the Primary Care Networks (PCNs). Focusing on strong communication and interpersonal skills, PFs are adept at building and maintaining trust, negotiating with stakeholders, and working to support continuous quality improvement (QI) processes.

The Practice Facilitator is responsible for the implementation of the Patient Medical Home program within our member clinics and PCNs. Aspects include panel identification, maintenance and management, CII/CPAR, ASaP screening and prevention programs; EMR standardization, advanced EMR usage, Initiative planning and evaluation, and other quality improvement projects.

What you will do...
  • Develop professional relationships with clinic teams, physician leaders/champions, PCN staff, and provincial peers/support programs
  • Collaborate with clinic staff to customize workflow processes for supporting PMH implementation activities
  • Understand clinic team goals for improvement and connect their ideas to the PMH model while helping those teams to generate, interpret, and analyze practice-level data (qualitative and quantitative) to measure baseline and improvement
  • Introduce provincial quality improvement opportunities, programs, and supports to clinic teams
  • Build quality improvement capacity in clinic teams and other PCN staff members
  • Facilitate PCN measurement and evaluation needs with clinic teams and around Initiatives
  • Support CII/CPAR participation within member clinics
  • Provide all required data and reports to the Executive Director

What you need to succeed...
  • A minimum of 1 year of experience is required in the healthcare field
  • Preference will be given to candidates with previous experience as a coach, mentor, or facilitator
  • Ability to work within tight deadlines and prioritization of work to achieve them
  • Strong analytical, critical thinking, troubleshooting, problem-solving skills, and a high degree of accuracy and attention to detail
    Superior written and verbal communication and interpersonal skills
  • Ability to work independently under minimal supervision, as well as the ability to work cooperatively in a multi-disciplinary team environment
  • Advanced technical proficiency in Microsoft Office Suite (e.g. Excel, Word, PowerPoint, Outlook)
  • Preference will be given to candidates with EMR experience (Accuro specifically)
