Position Summary

The Dietitian’s primary function is to interpret and communicate current scientific information regarding food and nutrition.  The Dietitian will assess, develop, implement, and evaluate nutrition care plans for physician referred patients, and provide both direct and indirect supports.

What you will do...
  • Complete initial patient assessments and develop nutrition care plans
  • Maintain timely and accurate documentation in the Electronic Medical Record (EMR) in accordance with practice standards and organizational policy
  • Facilitate development of patient goals; document and monitor progress towards goals
  • Support patients through development of self-managed nutrition plans and provide follow up to assist patients in achieving their individual health goals in a safe and appropriate manner
  • Participate in multidisciplinary treatment planning and patient conferences
  • Assist in meeting the self-management needs of patients through collaboration and consultation with members of the multi-disciplinary team
  • Communicate with team members, physicians, and other healthcare professionals regarding patient outcomes
  • Develop evidence-based educational resources and tools
  • Develop and facilitate patient education programs
  • Collaborate with community agencies as required
  • Deliver community education sessions
  • Connect patients to appropriate community resources and services
  • Maintain professional requirements and ongoing active participation in professional development
  • Maintain current knowledge and skills in clinical practice
  • Participate in program development and quality improvement activities
  • Knowledge of and adherence to organizational policies and procedures
  • Offer training support for other team members and assist in their professional development
  • Acts as a resource to other PCN staff
What you need to succeed...
  • Registered with College of Dietitians of Alberta (or eligibility)
  • University degree in Nutrition or equivalent
  • Completion of an accredited internship program
  • Graduate level training in public health would be an asset
  • Fluency in English language (written and verbal)
  • Computer skills: email, MS Word, MS PowerPoint, data entry, scheduling
  • Demonstrated ability to interpret and convey nutrition requirements and recommendations across the life span and in practical ways to different audiences
  • Effective teaching and facilitation skills
  • Effective counselling skills in individual and group settings
  • Awareness of social determinants of health and principles of self-management
  • Ability to apply behavioural health change strategies in practice
  • Ability to work cooperatively with physicians and clinic staff in a multidisciplinary setting
  • Ability to build a good working relationship with patients, families, and other healthcare professionals
  • Ability to make evidence-based decisions
  • Knowledge of current clinical practice issues
  • Ability to monitor clinical standards
  • Ability to work with diverse populations
  • Ability to thrive and contribute in a changing environment
  • Ability to occasionally work outside of normal business hours
